New Patients

If you wish to register with our Practice as a permanent patient please ask at Reception. If you are within our catchment area and complete a New Patient Registration form – GMS1 and Registration Questionnaire.

Please click on both downloads to complete and return to practice. We are unable to accept these forms electronically.

You will be registered with a named GP but you may request an appointment with any GP at the practice. It may not always be possible to see the GP of your choice on a particular day but pre-bookable appointments are available.

If you have a new baby, it is in the best interests of the baby to be registered with a GP as soon as possible. You will need the babies NHS Number which will be in the Red Book.
