Change from Edoxaban to Apixaban

We are reviewing anticoagulant (“blood thinner”) medicines across the practice in line with updated NHS/ICB prescribing guidance.
As part of this, we will be changing your anticoagulant from Edoxaban to Apixaban. Apixaban is a well-established anticoagulant used widely across the NHS and has several benefits, including a strong safety profile and flexibility around food/meals. IMPORTANT: Unlike Edoxaban, which is taken ONCE daily, Apixaban must be taken TWICE daily, approximately 12 hours apart. Taking it regularly is very important to ensure you remain protected against blood clots.
When switching:
• Take your final dose of Edoxaban as normal.
• Start Apixaban 24 hours later, at the same time the next day.
• From then on, continue Apixaban twice daily (12 hourly)
