  • Tablet form (for example Apixaban/Edoxoban) which requires regular blood tests every (3, 6 9 or 12 months) at the surgery with our phlebotomist. Your blood results will be reviewed by a clinician and you will be contacted if the clinician feels that you need to amend your dosage.

Chronic Disease Monitoring

All patients who have had a diagnosis of any of the conditions listed below will be offered a review at least annually with a doctor or a nurse. The practice will send you a letter or text every year asking you to attend a clinic.

For many of these conditions you will need to have a blood test at least a week before your clinic appointment. You will be given details when you are contacted. Please give the practice your email address and mobile phone number if you have them, so that we can communicate with you as efficiently as possible.
