Adult Social Care
Adult Social Care in Wirral are an integrated health and care provider which means different health and care professionals work closely together to support individuals to achieve their personal goals, making sure the right advice and support is provided at the right time from the right people. These include:

Any adult, aged 18 or over, who is finding it difficult to perform some day-to-day tasks, may qualify for care and support. They might be struggling as a result of age, illness, disability or any significant change in their life.
The aim of adult social care services is to support people to stay independent for as long as possible and maintain their wellbeing. This may include accessing support from a range of community, voluntary and advocacy services.

Wirral Community Health and Care Services
Wirral Community Health and Care Services offer a wide range of community health and social care services for adults and older people in Wirral. These include community nursing, adult social care, therapy services, cardiology services and much more.
Some services require a referral from your GP or other health professional responsible for your care e.g. hospital consultant. Others you can refer yourself including; Bladder and Bowel Service, Community Nursing, Diabetes Smart course, Parkinson’s Disease Nurses.
