If you wish to register with our Practice as a permanent patient you can now do this online using the link below.

If you are unable to register online and are within our catchment area and complete a New Patient form – Available here Registration form – 1.11.24

You will be registered with a named GP but you may request an appointment with any GP at the practice. It may not always be possible to see the GP of your choice on a particular day but pre-bookable appointments are available.

If you have a new baby, it is in the best interests of the baby to be registered with a GP as soon as possible. You will need the babies NHS Number which will be in the Red Book.

Catchment Area (Practice Boundary)

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

From 1st April 2015, GP Practices are required to allocate every patient with a named GP. This does not affect your treatment or care at the Practice and you can continue to see any GP of your choice as normal.

If you are unsure of whom your named doctor is please ask a receptionists.

Patient Online Access

We now offer patients access to their records. This includes allergies, medications, test results, immunisations and documents.

If you would like to request online access to your records please ask reception for a form and an information leaflet. If you already have online access but would like the access please complete an online missing information form and hand it in to reception.

Alternatively print the attached forms to complete the requests which can then be handed in to reception

The access will only be given from the date of registration

In order to receive a registration form and to gain access to your medical record you will need to provide two form of identification one of which must be photographic
